Update #14: Medical waivers. Please note May 23 deadline!

Dear Team Iowa members:

Two more things you need to be aware of:

(1) When you register, the “athlete” should receive a medical waiver form. This needs to be filled out by a physician that you see on a regular basis – it does NOT need to be filled out by your transplant surgeon. You can NOT have this filled out before March 11, 2008. YOU NEED TO RETURN IT TO BRUCE HOFFMASTER BY MAY 23, 2008. He has to compile them for the whole team and forward to national NKF by early June. THIS IS A NEW DEADLINE FOR YOU! Please check your calendar and make an appointment to get this signed after March 11th and before May 23rd. DO NOT WAIT UNTIL THE LAST MINUTE – You can’t participate in the Games without this.

(2) When you register as a non-athlete, if you receive a medical waiver form, please disregard it – there has been a glitch in the registration system and some people are reporting that they are receiving waivers when they are not athletes. The only people that need medical waivers are the athletes – the people that have received a life saving organ transplant and are planning to participate in the Games’ athletic events.
